WHAT WE DO:

HOW WE DO IT:

We will do this by striving to create an environment where choice, and empowerment will dictate the focus for the people that we support.

WHO:

PBAL Group is a new agency approved by DDA to provide community residential services to people with intellectual and developmental disabilities in the state of Maryland.

WHAT:

PBAL Group will provide Community Living-Group Home Services to assist individuals to acquire skills necessary to maximize their independence in activities of daily living and to fully participate in community life.

WHERE:

The Community Living -Group Home will be in an integrated settings within Prince George’s County where supports to full access to greater community is ensured for no more than 3-4 people living in an apartment and/or a house. This includes where opportunities to seek employment and work in competitive and integrated settings, engage in community life, control personal resources, and receives services in the community to the same degree as individuals who do not receives services from Medicaid Waiver.

  • The residential setting will be selected by the individual from among settings options; where each individual has the right to privacy, to be treated with dignity, and respect and free from bullying and restraints.
  • Where each individual is supported in making choice
  • Where each individual is given choice regarding services and who provides them.

 

WHY:

It is the passion of Pbal Group founders to serve individuals with intellectual and developmental disabilities; to honor and respect God’s given rights for all to live freely anywhere in the community.

 

SCOPE OF SERVICES:

DDA-Community Living- Group Home:

PBAL GROUP: will support individuals with intellectual and developmental disabilities to gain skills to maximize independence with activities of daily living and to participate in community life of their chosen. Skills to be developed or maintained under Community Living Group Home Services will be determined based on the person’s individualized goals and outcomes as documented in his or her person centered plan. Examples of this service includes:

  • Learning social acceptable behavior
  • Learning effective communication
  • Learning self-direction and problem solving
  • Engaging in safety practices
  • Performing household chores in safe and effective manner
  • Performing self-care
  • Learning skills for employment
  • Developing new relationships and natural supports

This service will also include coordination, training, supports, or supervision as specified in the person’s PCP and the availability of back-up and emergency support 24 hours a day. Community Living Group Home Services are usually provided to no more than (4) individuals in one home. Any exceptions require approval by DDA. Community Living Group Home Services also include trial experiences for people transitioning from an institutional or non-residential sites on a temporary, trial basis.

 

SUPPORT & SERVICES:

PBAL GROUP will serve individuals with the following disabilities:

Intellectual disability, mental health, autism spectrum disorder, Cerebral Palsy Muscular Dystrophy, Multiple Sclerosis, Blindness, Severe Visual Impairment Deafness, Severe Hearing Impairment Behavioral Problems, Epilepsy/Seizure Disorder, Head Injury, Orthopedic Impairment, Specific learning Disability, Speech/Language Impairment, Spina Befida, Spinal Cord Injury, Chemical Dependency, Cystic Fibrosis, Other Neurological Impairment, and other undetermined disabilities.

 

STAFFING & TRAINING:

PBAL Group will ensure that staff acquire all the relevant DDA trainings at all levels and patterns as determined in the PCP. Such trainings required shall embody health and safety credentials but not limited to: CPR, First Aid, Blood Borne Pathogens, Fire Safety, Communicable Diseases, Medication Administration (MATP), Fundamental Rights, Community Integration, Supporting Individuals, families and choice making etc.

In addition, staffing will be coordinated by the appropriate Manager within the program. The agency maybe utilizing the College of Direct Support, an online staff orientation program, within the fiscal year. The content of the courses includes the following subjects related to Community Learning Services: Safety at Home and in the Community; Supporting Healthy Lives; Individual Rights and Choice; Community Inclusion; You’ve Got a Friend: Supporting Family Connections, Friends, Love, and Pursuit of Happiness; Person Centered Planning and Supports; Employment Supports: Exploring Individual Preferences and Opportunities for Job Attainment; Cultural Competency; and Personal Care. PBAL Group staff will be expected to complete designated required and non-required training modules along with a specific time frame for mentoring before working alone with individuals in the community. The Mandt System Training for all Direct Service Professionals (DSP) will also be required for Pbal Group DSP’s.

 

SETTINGS & LOCATION:

PBAL GROUP: Community Living-Group Home services will be provided in a licensed operated setting to no more than (3-4) people in one apartment and/or a home within Prince George’s County, Maryland. Any exceptions will require approval by DDA. Community Living Group Home services will also include trial experiences for people transitioning from an institutional or non-residential sites on a temporary, trial basis. Pbal Group Community Living Group Home will be located in the community where there is opportunity to seek employment, engage in community life, control personal resources, and receives services in the community to the same degree as individual, who do not receive services from Medicaid waiver.
